Work that makes a difference!Pursuing Excellence, Inspiring Innovation, Promoting Teamwork, Embracing Service and Advancing EquityUnder direct supervision of the Community Systems CORE Team Lead, the health educator assures the coordination of community health assessments and development of community health improvement plans/activities, including activities impacting the social determinants of health (SDOH) in the various counties.
Plans and implements community- based Chronic Disease Risk Reduction interventions which address the health risk factors of physical inactivity, poor nutrition, tobacco use, obesity, cancer, diabetes , arthritis and other related chronic diseases.
Supports health behavior change at the individual, community, institutional and public policy levels.
Provides leadership in the assessment of state, regional, county health data, community resources and processes to determine at-risk populations and risk factors, including the social determinants of health (SDOH) to be addressed in their covered counties.
Facilitates the development of community health assessments and community planning to identify strategic issues and prioritize health problems.
Implements evidenced-based policy, systems, and environmental (PSE) change strategies directed toward community health improvement.
Provides consultation, and technical assistance to health department staff, community organizations, health care systems, programs, and local coalitions/task forces addressing population health and chronic disease risk reduction and COVID education and prevention for at-risk populations.
Establishes/maintains strong working relationships with stakeholders, including other programs, government agencies, and nongovernmental lay and professional groups to build support for chronic disease & COVID-19 prevention and control.
Uses effective community engagement strategies to facilitate coordination/collaboration and addresses chronic disease issues.
Leverage resources to impact prevention and create partnerships.
Evaluates the implementation and effectiveness of programmatic and community-based interventions through the collection/analysis of process and outcome variables.
Maintains and submits required reports to region leadership and program management as required in a timely manner.
Participates in required state, regional and program meetings/courses.
Participates in training to improve health education skills, maintains or develops content specific proficiency, and keeps abreast of changes in program standards, policies, and procedures including CHES/MCHES credentialing, if it applies.
Performs other related duties as assigned, including community outreach for special projects and/or public health emergencies.
Participates in disaster preparedness as outlined by the DHEC region/county plan during hazardous weather or declaration of emergency, subject to duty and/or call on a 24 hour basis as an essential employee.
Minimum and Additional Requirements State Minimum Requirements:
A bachelor's degree in health education, health promotion, or a closely related field such as psychology, biology, sociology, education, or physical education and health education experience.
Institutions of higher Learning must be recognized by the Council for Higher Education Accreditation.
Agency Additional Requirements:
Bachelor's Degree in Health Education, Public Health Education, School Health Education, Community Health Education, Health Promotion or a closely related field that confers CHES eligibility and two (2) years of health education experience; or a Master's Degree in the above and one (1) year of health education experience.
Knowledge of the principles, practices and theories of public health education and health promotion; ability to plan, organize, guide, and coordinate the work of others.
Ability to adapt/design and deliver presentations/curricula with sensitivity to learner characteristics.
Ability to influence evidenced-based policy, systems, and environmental (PSE) change.
Knowledge of the social determinants of health (SDOH) and the health issues of at-risk populations.
Ability to engage communities and establish /maintain effective working relations with public/private agencies, organizations and groups to advance community health.
Agency standard work hours are Monday-Friday (8:
30 am - 5:
00 pm).
Employee must be able to work flexible hours (late weekday evening and/or weekend) to meet the needs of the community.
Occasional overnight travel may be required.
Must have a current, valid driver's license and personal car available to travel to community sites within the Region.
Preferred Qualifications CHES/MCHES or CHES/MCHES-eligible is preferred.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
